From the alert to the scene.

Alerts fire automatically when a player goes down and again when they die, and a downed player can call for help with one key. With Oxide Dispatch installed the alerts land on a board; without it, manual calls still page on-duty medics directly.

  • Automatic down and death alerts drop on the dispatch board, debounced so one incident does not fire twice
  • Downed players press G for a distress signal, and anyone can send /911e with a message

Seven regions. Real bones. Live vitals.

Examine a patient and a scripted camera frames them, face-on if they are standing and top-down if they are on the ground or an operating table. Seven clickable dots track real ped bones; click one and the overlay tells you what is wrong there.

  • Head, torso, abdomen, arms, and legs; injuries group onto the nearest region, illnesses onto the torso
  • Each region lists what is wrong, how bad it is, the price, the treatment time, and the supplies needed
  • Supplies the medic is not carrying are flagged red on the condition card
  • Blood type and live vitals: pulse, blood pressure, blood oxygen, and a cardiac-arrest flag

Critical patients don't get up in the street.

Severe injuries mark a patient critical, and critical patients cannot be revived on the street. Stop the bleeding, stabilize, then transport to a hospital bed, where the medic's check-in is the revive.

  • Gunshots and lacerations are the only treatments available in the street; everything else is a facility job
  • Bleeding must be treated before a stabilization takes, and a fresh wound cancels it
  • Stabilizing freezes the bleedout timer, holds for 10 minutes by default, and blocks respawn under care
  • Load patients into an ambulance seat, or onto the stretcher, which loads into the rear with them still on it

Scan it, then operate on it.

Place an x-ray screen on the wall and the scan appears there for everyone nearby. The operating-table monitor shows the record and an ECG driven by the patient's actual pulse. Every treatment, in the field or on the table, runs as the same graded keypress minigame, with a cap on how much harm one patient can take.

  • Scans show fractures and retained bullets only, broadcast to every screen within 30 meters
  • Retained bullets and fractures at severity 3 or higher are surgical, so they need an operating table
  • 85% or better cures clean, 50 to 85% cures with a complication, and below 50% fails and adds a laceration
  • Sequences are generated and re-scored on the server, so a client cannot report a run it did not play

The chart follows the patient.

Oxide EMS reads and writes the oxide-medical record: blood type, injuries, illnesses, fractures, and vitals. Severity decides who is critical, and a bed revive gets a patient back on their feet without erasing what put them down.

  • Any active condition at severity 4 or above marks the patient critical, read straight from the record
  • Bed revives restore health but leave injuries in place by default, so surgical injuries still need a medic
  • Doctor's offices take walk-ins: a patient rings the reception bell and on-duty staff are paged
  • Public supply-shop rows let civilians buy their own cures; rank-gated rows are staff only

Built in-game, saved to your database.

Departments, grades, facilities, garages, the fleet, uniforms, radio channels, and the supply shop come from /ems admin, not a config file. Every interaction point is placed by walking to the spot, and bosses run roster, treasury, payroll, and fleet from a management desk.

  • Hospitals get beds, a respawn point, x-ray machines, and operating tables; doctor's offices get a reception
  • Beds place with a ghost patient lying in position; x-ray screens and monitors are resizable preview quads
  • Uniforms capture what you are wearing and own only the clothing slots you tick
  • Every setting is edited live in /ems settings, stored in the database, and applied without a restart
